Sue and Johnsy are two young actors who share a small flat on the third story of an old house. Once in November, Johnsy falls very seriously ill. He has pneumonia. Soon she gives up hope of survival. The doctor treating her does not see any positive change in her condition.

One day he tells Sue that Johnsy's chances of survival are limited unless she has something to hope for. She has made up her mind that she is not going to get well. If she loses hope of living, drugs will do nothing. Sue does her best to keep Johnsy interested in the things around her.

But there is no response from Johnsy's side. She always lies on her bed and watches through the window an ivy plant that is slowly losing its leaves, and has contemplated in her mind that it will die when the last leaf falls.

Sue continues to explain to Johnsy that it is foolish to pin her fate to the survival of the last leaf on the vine. Old ivy leaves have nothing to do with its recovery. The doctor is confident that she will be fine. Johnsy is too sad to say anything.

She keeps counting the remaining leaves on the creeper. One day Sue informs an old fellow cast member Behrman, their downstairs neighbor, and she is furious that Johnsy has so little hope. He is aware of his desire to die when the last leaf falls.

Behrman comes to their room and finds Johnsy sleeping. Sue pulls the curtain together and they go to the next room. She glances out the window and sees only one leaf on the creeper that seems to fall anytime because it is raining heavily and an icy cold wind is blowing.

Behrman doesn't say a word. He goes back to his room and decides to do something for Johnsy's life. He paints a similar leaf and sticks it on the creeper while Johnsy is sleeping. But he lost his life working in the open during extreme cold and heavy rain. He dies of pneumonia.

The next morning, after a severe storm, Johnsy sees the last remaining leaf still clinging to the vine. She is full of hope. She decided that she wanted to continue living. She thinks there must be some reason why Patti has refused to die, and that it is a sin that she wants to die.

Johnsy soon recovers from her illness.

Not long after, Sue informs Johnsy that Behrman had died of pneumonia, which had occurred while outside in the wet and cold, painting the last leaf. Behrmann had painted his long-promised masterpiece—the leaf that saved Johnsy's life, sacrificing himself in the process.
